To register at this practice you need to live in the practice area (see below).
You will need to complete a registration form which you can print off here and return to the practice, or you collect one from the practice reception. There will be additional forms to complete including a Personal History Questionnaire. Ideally, you will need your NHS number to assist in completing the forms.
If you have an NHS Medical Card please bring it with you to the practice when completing your forms.

The villages of Nettleham, Scothern, Barlings, Stainton by Langworth, Newball, Sudbrooke, Riseholme, Grange de Lings, North Greetwell, Cherry Willingham, Reepham, Fiskerton, Short Ferry and Hawthorn Road (CW).
The Practice boundary also includes the Carlton Estate, bounded by Bunkers Hill and Outer Circle Road.
