Patient Registration

To register at this practice you need to live in the practice area (see below Practice Area Map). 

To register, you will need to complete a GMS1 form and a personal history questionnaire. You can complete the GMS1 form by clicking on the link below. Once completed, press the submit button and the form will be sent via email to the practice. Once we receive your form, we will then start the registration process. We will then send you either a text or an email with the link for the personal history questionnaire. Please complete the form and click submit and the responses will come back directly to the practice. Once we have processed this form, we will send you a text to confirm that you are fully registered with Nettleham Medical Practice.

If you do not have internet access, please call into either of the practice sites to collect the appropriate forms which can completed by hand.

The villages of Nettleham, Scothern, Barlings, Stainton by Langworth, Newball, Sudbrooke, Riseholme, Grange de Lings, North Greetwell, Cherry Willingham, Reepham, Fiskerton, Short Ferry and Hawthorn Road (CW).
The Practice boundary also includes the Carlton Estate, bounded by Bunkers Hill and Outer Circle Road.
