Please include our practice code C83031 as you will be required to enter this as the mandatory ‘Voucher code’. You will also need to enter their NHS number to request a self-swab kit – you can find this on any hospital letter or the NHS app.

Everyone can take part – there is no upper or lower age limit but we do ask you limit it to one kit per household.
