Social Prescribers can work with patients to identify potential solutions to difficulties they face on a day to day basis. They explore where you are in life currently as well as where you would like to get to. They then help you identify the barriers to living the life you want. They also work with hundreds of charities and other organisations who can provide specialist input if required.

At Nettleham
The NHS Health Check is a health check-up for adults in England aged 40 to 74. It’s designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. As we get older, we have a higher risk of developing one of these conditions. An NHS Health Check helps find ways to lower this risk.
It is not available to patients with pre-existing conditions, as we monitor those as part of the annual review system.

All patients with selected long term conditions and / or medication on their repeat medication list are offered an annual review. You will be called in once a year by our reception team. However, if you have problems with your medications you can always contact one of our Clinical Pharmacists at the surgery, or your community pharmacist. Similarly, if you have any problems with your health conditions between reviews, please do let us know via askmyGP. You can also access help and support for your longterm conditions on this website.

Anyone who is suffering from hearing loss can book in with one of our nurses who will examine the ears for wax and other common causes of hearing loss. If they don’t find any reason for the hearing loss, they can arrange onward referral for an NHS hearing test at the local hospital. To arrange an appointment, please send us a message via askmyGP.

We are pleased to provide a fully comprehensive family planning service to our patients and those of surrounding practices too. We can provide advice and prescriptions for all types of contraceptive pill, as well as fit coils, mirena coils, and implants. To arrange a consultation, please send us an askmyGP via the website. If you’re registered at another practice, you will need to ask them to arrange a referral to us.

We have a practice diabetes team who offer routine check ups as well as consultations for any difficulties our diabetic patients face. The current team includes:
